Instructions

- Check the optional business-records statement only if it is true.
- The jurat requires a genuine oath or affirmation before a notary public. The custodian signs in the notary’s presence.
- Not a substitute for special court-production procedures under G.L. c. 233, §§ 79 or 79J.
